Jordan Valley approved to offer program for older adults

Jordan Valley Community Health Center has received official approval to operate the Program of All-Inclusive Care for the Elderly in the Springfield area.

The Springfield health system is the first in southwest Missouri and third in the state approved to provide the PACE program, according to a news release. PACE includes medical and personal care, rehabilitation, social interaction, medications and transportation, with the goal of allowing patients to continue living at home.

The program, which is operating locally as Jordan Valley Senior Care, represents a "three-way agreement" between Jordan Valley, MO HealthNet and Medicare, said Ryan Kruger, the health system's vice president of operations, via email.

“PACE is perfectly aligned with Jordan Valley’s mission to improve access to quality health care for the underserved,” JVCHC President and CEO Dr. Matt Stinson said in the release. “The PACE model also has a strong track record of high-quality outcomes for participants, including lower hospitalization rates and reduced emergency room visits."

Jordan Valley Senior Care is currently launching an assessment and enrollment process, with plans to start caring for participants in July. PACE enrollment is voluntary, and patients must be at least 55 years old, live in the PACE service area, be certified by the state as meeting the need for the nursing home level of care and be able to live safely in the community with the help of PACE services, according to the release.

"PACE improves access to health care for senior participants to prevent more serious and costly issues downstream," Kruger said in the release. "Families and caregivers for PACE participants are largely supportive of the model. Research found that 95% of family caregivers would recommend PACE to someone in a similar situation."

Branco Enterprises Inc. recently served as general contractor for a 12,250-square-foot space for PACE within Jordan Valley's 1720 W. Grand St. clinic, according to past reporting.

Kruger said via email that the clinic at Grand and Kansas Expressway will serve as home base for Jordan Valley Senior Care.

“Our current facility can accommodate up to 300 participants, but we plan to grow with the demand,” he said.
