Citizens Memorial Hospital has opened a new clinic in Bolivar.
The CMH Health & Wellness Clinic, located at 2520 S. Springfield Ave., Ste. A, opened to patients on March 4, according to a news release. A ribbon-cutting ceremony is scheduled April 4.
The clinic provides wellness visits for patients who do not have a primary care provider, as well as hospital transitional care services for patients without primary care support.
"The hospital transitional care services will provide post-discharge care to CMH’s highest-risk patients who are discharged from inpatient care or the emergency department. We’ll be able to help connect those patients to a primary care provider," said Michelle Boehne, administrative director of clinics at CMH, in the release. "Additionally, we’ll be able to provide the Medicare annual wellness visit to patients without a PCP and get them started on the road to routine health and wellness care.”
Family nurse practitioner Shelby Bailey, who has past experience as a family nurse practitioner hospitalist at Mercy Hospital Springfield, is leading the new CMH clinic. The clinic does not provide walk-in services, health system officials say, pointing to the CMH Walk-In Clinic at 2230 S. Springfield Ave., Stes. H-J.
CMH runs 34 primary care and specialty physician clinics in Ash Grove, Bolivar, Buffalo, El Dorado Springs, Greenfield, Humansville, Osceola, Pleasant Hope, Stockton and Willard, according to the release.
