National; Critical violations found: 5. *Observed employee put raw patties on grill with gloves on and then grab fresh buns with same gloves, buns were held up against his shirt while transferring to toaster. *Creamers stating “keep refrigerated” were being held at 80 degrees over pan of melted ice. *Cooked onions sitting out on counter not being kept hot or cold. (Policy is to use time as the control but time four hours in the future had not been documented.) *Metal stem thermometer contained sticky residue while stored in drawer. *Leak under three-vat being contained in bucket. Noncritical violations found: 1.
