City Restaurant Health Inspections

Listed here are the restaurant health inspections conducted by the Springfield-Greene County Health Department. Restaurants are inspected for critical and noncritical violations of state standards. According to the Springfield-Greene County inspection code, critical violations are “more likely than other violations to contribute to food contamination, illness and/or injury.” And noncritical violations, “although important to the overall food hygiene of the establishment, do not necessarily present an immediate threat to food contamination, illness and/or injury.” The number of both critical and noncritical violations are given and specific critical violations are listed. *Denotes a critical violation.

April 10–14, 2006

The Acoustic Cafe, 434 S. Jefferson; Reinspection. Corrections: spring traps in place, three mice caught. Owner of building working on making repairs to north side of building, soda nozzles clean and all chemicals have been picked up off the floor under three-vat for easier cleaning and pest control management. Critical violations found: 0. Noncritical violations found: 0.

Bambu Vietnamese Cuisine, 1338 E. Battlefield; Critical violations found: 0. Noncritical violations found: 4.

Bens Carriage House Sandwich Shop, 1772 S. Glenstone; Reinspection. All critical violations corrected, can opener and shake mixer heads clean and sanitized, sanitizing solution at proper strength, chicken stored in designated area away from raw vegetables, cook’s hand sink waste line is repaired. Critical violations found: 0. Noncritical violations found: 3.

Cartoons, 1614 S. Glenstone; Critical violations found: 4. *Adulterated food found on shelf. Discarded. *Big Room - All soda guns not clean (clean and sanitize daily). *Can opener not clean (clean and sanitize daily). *Leak under cook’s hand sink waste line (maintain in proper repair). Noncritical violations found: 7.

CBC Snack Bar - Zee’s, 3000 N. Grant; No violations found.

Central High School, 423 E. Central; Reinspection. No violations found.

Chick-Fil-A, 2825 S. Glenstone; Critical violations found: 2. *Chicken dicer contained dried-on chicken last used yesterday. *Mold observed up inside ice machine around chute area. (Clean and sanitize when machine is empty.) Noncritical violations found: 0.

Drury University Cafeteria Commons, 900 N. Benton; Critical violations found: 2. *Basil mayonnaise in walk-in cooler not dated. (Requires refrigerated, ready-to-eat potentially hazardous food prepared and held for more than 24 hours to be clearly marked to indicate the date by which food shall be consumed. Corrected by disposal.) *Spoon in clean equipment not clean. (Requires food-contact surfaces to be clean to sight and touch.) Corrected by rewashing spoons. Noncritical violations found: 7.

Frisco Crossing, 3253 E. Chestnut; Critical violations found: 0. Noncritical violations found: 2.

Gem Of India, 211 W. Battlefield; restaurant voluntarily closed until cleaning issues and pest management issues can be corrected. Critical violations found: 8. *Lack of demonstration of knowledge by the person in charge based on critical violations cited. *Temperature of two-door freezer 40 degrees, box of breaded mushrooms sitting in pool of blood in bottom of freezer. All food discarded. *Hot foods are not being routinely held at 140 degrees when the curry potatoes on the buffet were 130 degrees and the cooking rice was 100 degrees. Food discarded. *No dates observed on any ready-to-eat foods made in-house. *No sanitizer in final rinse of dish machine. (Owner called dish machine chemical company to make repairs.) *Used ice cream scoop sitting on shelf over freezer, dried on ice cream product, scoop has not been used today. *More than 100 dead roaches found throughout the kitchen, bar and buffet area. Live roaches observed on sneeze guard, on food prep table, in bus tub, in refrigerator door gasket. Pest control has documented treatments two times a month. (Discussed treatments with pest control operator and cleaning issues with restaurant owner.) *Chemicals and medicines on same shelf with food. Noncritical violations found: 7.

Glendale Baseball Concession, 2727 S. Ingram Mill; No violations found.

Gracie’s, 1949 E. Sunshine; Critical violations found: 0. Noncritical violations found: 2.

Grove Deli, 1001 E. Primrose; Critical violations found: 0. Noncritical violations found: 2.

Hillcrest Baseball Shack, 3319 N. Grant; Leasa Light, cook at Truman School, will take utensils to Truman to wash, rinse, and sanitize. No violations found.

Hinode Restaurant, 4301 S. National; Discussed pest prevention and plumbing maintenance. Critical violations found: 0. Noncritical violations found: 2.

Jade Dynasty, 300 E. Battlefield; Discussed importance of keeping steam tables at the right temperature, so as to keep hot foods holding at or above 140F. Critical violations found: 3. *Fried chicken on buffet found holding at 100F. (Maintain hot holding temperatures at or above 140F.) Corrected by discarding, cooking new to 165F, adjusting steam table temperature and holding above 140F. *Ground beef in cooler not properly date labeled with a seven-day discard date. (Label all potentially hazardous, ready to eat food with a seven-day discard date.) Corrected by voluntarily discarding. *Sanitizer water for wiping cloths was noted at 0 ppm. (Maintain 50-100 ppm concentration of sanitizer.) Corrected on site. Noncritical violations found: 3.

Kickapoo High School, 3710 S. Jefferson; Critical violations found: 1. *Sliced ham on steam table noted at 135F. (Maintain all potentially hazardous hot foods at or above 140F.) Corrected by reheating to 165F and holding above 140F. Noncritical violations found: 1.

La Hacienda, 1370 S. Glenstone; Reinspection. Few critical violations corrected, cutting boards replaced with smooth, easily cleanable ones; all steam table foods reheated properly. Critical violations found: 4. *Eating behind cook line by chip fryer, drinking from open cup (educated staff on policies of eating in designated areas away from food prep and drinking from a closed container with lid and straw). *Raw chicken stored in the same container as raw beef (Chicken must always be stored separate from other meats due to requirements of higher heat needed to kill pathogens). *Duplicate: Inside ice machine not clean. (Must be clean and sanitized in all areas of the ice machine.) * Duplicate: Vegetable dicer not clean. (Must be cleaned and sanitized daily.) Noncritical violations found: 2.

La Hacienda, 1370 S. Glenstone; Reinspection. All prior critical violations corrected, raw chicken stored separate from other raw meats and ready-to-eat foods, vegetable dicer clean and sanitized, inside ice machine clean and sanitized. Critical violations found: 1. *Sanitizing solution too strong at 200ppm. (corrected at 50-100ppm). Noncritical violations found: 2.

La Quinta, 2535 S. Campbell; Reinspection. Corrections: hand sink is draining properly and is equipped with soap and paper towels. No violations found.

Lucy’s Chinese Food, 3330 S. Campbell; Discussed proper cooling methods for potentially hazardous foods. Critical violations found: 2. *Person in charge unable to demonstrate knowledge of proper cooling technique, correct way to wash/rinse/sanitize and proper concentration of sanitizer. *Sanitizer concentration noted below 50ppm. (Maintain between 50 and 100ppm.) Corrected on site. Noncritical violations found: 7.

Mandarin Inn, 706 E. Battlefield; Discussed importance of utilizing test strips to monitor proper concentration of sanitizer solution. Critical violations found: 1. *No sanitizer found in sanitizing solution for wiping cloths. (Provide 50 ppm-100 ppm concentration of bleach for wiping cloths.) Corrected on site. Noncritical violations found: 4.

McDonald’s, 2512 E. Sunshine; Critical violations found: 1. *Inside ice machine deflector not clean (clean and sanitize). Noncritical violations found: 1.

Mr. Yen’s, 4117 S. National; Discussed frequency of cleaning and pest control methods. Critical violations found: 1. *Presence of roaches noted in food prep area. (Contact pest control to eliminate this problem and clean nonfood contact surfaces regularly for pest prevention.) Noncritical violations found: 6.

Mr. Yen’s, 4117 S. National; Reinspection: Revisited site, to note that all noncritical violations have been corrected. On Wednesday of this week, general manager John Burke had his pest control company treat the facility. Since no receipts were left to denote what treatment was done, I called them while I was at the facility. John’s pest control company told me where they treated and the chemicals he treated with. He said that the type of chemical used would not kill all the roaches immediately, but instead kill them over the course of a couple days, as the spray has time to affect them. Since there weren’t as many roaches on this reinspection, I decided to allow John to have the weekend to get rid of all the roaches. A follow-up inspection will be done to see that the pest control chemicals have had plenty of time to kill the remaining roaches. If a presence is still noted on Monday, then further action with the pest control will be taken. Critical violations found: 1. *Presence of roaches still noted in food prep. area. (Contact pest control.) Noncritical violations found: 1.

Northview Senior Center, 301 E. Talmage; No violations found.

Pepperdine Learning Center, 1518 E Dale; Critical violations found: 0. Noncritical violations found: 2.

Pipkin Middle School, 1215 N. Boonville; Heat sanitizing dish machine required two passes to obtain 160F on surface of tray. No violations found.

Robberson School, 1100 E. Kearney; No violations found.

Scramblers, 3653 E. Sunshine; Critical violations found: 1. *Inside ice machine chute not clean (clean and sanitize). Noncritical violations found: 6.

Soo’s Korean Restaurant, 3644 S. Campbell; No violations found.

St. George Donuts, 3628 E. Sunshine; Critical violations found: 1. *Small refrigerator not cold holding adequately 58F (must cold hold at 41F or below). Noncritical violations found: 4.

St. John’s Springfield Clinic-Smith Glynn Branch, 3231 S. National; No violations found. Taco Bell, 4140 S. Lipscomb; No violations found.

Taco Bell, 601 S. National; Reinspection. Corrections: pans appear cleaner, continue to monitor closely. Critical violations found: 0. Noncritical violations found: 0.

Thai House, 2811 S. Campbell; Critical violations found: 6. *Owner left shortly after inspection began, no one in charge with food safety knowledge on cooling, hot holding temperatures, cold hold temperatures. No one would sign inspection sheet. *Observed two kitchen employees enter kitchen from out front through swinging door, did not stop to wash hands at hand sink right inside door before working in kitchen. *Cook prepared head lettuce for salad this morning, no disposable gloves on the premises. Observed server move spring roll from one plate to another with bare hands. *Mung bean sprouts held at room temperature 70F (discarded). These will be cooked but are potentially hazardous and need to be stored cold between use. *No dates on any cooked ready-to-eat foods. *Can opener blade contained build-up of food debris. Noncritical violations found: 11.

Trolley’s Downtown Bar & Grille, 107 Park Central Square; Critical violations found: 1.

*Two pans in walk-in cracked and not easily cleanable. Lid to alfredo sauce in steam table cracked, melted and not easily cleanable. (Requires food-contact surfaces to be smooth and easily cleanable.) Corrected by putting food in other containers and disposing of damaged containers. Noncritical violations found: 4.

Waffle House, 4128 S. National; Critical violations found: 1. *Eggs found stored above ready to eat lettuce. (Store potentially hazardous raw foods separate or below ready-to-eat foods.) Corrected on site. Noncritical violations found: 1.

Weller School, 1630 N, Weller; No violations found.

Casey’s General Store, 410 Miller, Willard; Critical violations found: 1 *Sanitizer too strong. Bottle of spray bleach has a concentration of greater than 200 ppm. Employees can use this on floors and walls only if it is labeled “For floors and walls only”, and only when there is no food preparation going on in the kitchen. Noncritical violations found: 1.
