City Restaurant Health Inspections

Listed here are the restaurant health inspections conducted by the Springfield-Greene County Health Department. Restaurants are inspected for critical and noncritical violations of state standards. According to the Springfield-Greene County inspection code, critical violations are “more likely than other violations to contribute to food contamination, illness and/or injury.” And noncritical violations, “although important to the overall food hygiene of the establishment, do not necessarily present an immediate threat to food contamination, illness and/or injury.” The number of both critical and noncritical violations are given and specific critical violations are listed. *Denotes a critical violation.

Oct. 16-20, 2006

The Acoustic Cafe, 434 S. Jefferson; Critical violations found: 2. *Open bowl of soup and drink on cook line; did not observe employee eating or drinking. Removed when pointed out. *Mouse droppings observed in back area around canned goods, gap at back door, one spring trap observed but not set. Noncritical violations found: 4.

Cravin Pizza, 3641 E. Sunshine; Critical violations found: 3. *Chemicals stored next to food products. (Chemicals moved to designated area.) *Cook eating in the kitchen (Educated staff on designated area to eat.) *Can opener not clean (Cleaned and sanitized.) Noncritical violations found: 3.

Dairy Queen, 3665 E. Sunshine; Critical violations found: 1. *Hot water in hand sinks throughout facility at 86 F. (Must meet peak hot water demands at 110 F.) Noncritical violations found: 4.

Dairy Queen, 3665 E. Sunshine; Reinspection. All violations corrected, all hand sinks have adequate hot water. Critical violations found: 0. Noncritical violations found: 0.

Fast -N- Friendly Deli, 3015 S. Scenic; Reinspection. Hamburgers were 149 F at 12:35 p.m. Critical violations found: 0. Noncritical violations found: 0.

Food 4 Less, 313 E. Battlefield; Critical violations found: 3. *Store added cooked chickens 10/1; Employee did not know hot-holding temperatures, cooling times and temperatures. Switched from quat sanitizer to bleach; did not know proper concentration or that quat test strips would not work for bleach. *Bleach in sanitizer sink greater than 200 ppm. (Refilled sink and measured out the correct amount of bleach.) *Metal stem thermometer used to check chickens contains dried-on residue. Noncritical violations found: 2.

Glendale Football Concession, 2727 S. Ingram Mill; Critical violations found: 0. Noncritical violations found: 0.

Hampton Inn South, 3232 S. Stewart; Critical violations found: 1. *Sanitizing solution too strong – 200 ppm. (Corrected at 50-100 ppm.) Noncritical violations found: 0.

Hanna Banana’s Gourmet Ice Cream, 303 E. Battlefield; Critical violations found: 0. Noncritical violations found: 0.

Holiday Inn Peppertree Restaurant, 2720 N. Glenstone; Reinspection. Placard placed by buffet informing customers of additional risks of consuming undercooked eggs. Dish machine was working properly at this time. Slicer was clean. Critical violations found: 0. Noncritical violations found: 1.

Hong Kong Inn, 1632 E. Sunshine; Critical violations found: 3. *Employee entered kitchen after eating lunch, rinsed hands at three-vat and wiped hands on rag before putting on gloves. (Owner had her remove gloves and wash hands at hand sink, gloves were discarded.) *Stir-fried chicken and beef cooked yesterday out of refrigeration at 46 F and 56 F. (Went over possibility of using time as the control for this product.) *Reusing milk jugs for pot sticker sauce. (Not designed for reuse and not easily cleanable.) Noncritical violations found: 0.

Hong Kong Inn, 1632 E. Sunshine; Reinspection. Corrections: cooked chicken and beef now being held in refrigerated make table, employees observed washing hands as necessary. New washable jugs purchased for food storage. Critical violations found: 0. Noncritical violations found: 0.

Jade Dynasty, 300 E. Battlefield; This inspection was prompted by a complaint that was sent to our office on 10/19/06 when customer noticed dead insects in the crispy noodles. He also wanted to complain about the hand sink in the Mongolian grill that had a continuous leak. Orkin pest control was here on 9/19/06. The pest problem at this facility appears to be under control at this time. Critical violations found: 3. *Potentially hazardous food items are not always held above 140 F or below 45 F on the buffet. Chicken on a stick 116 F, fried chicken 114 F, spring roll 90 F, custard 49 F. (Utilize time as a control with a documented policy and log.) *Interior of ice machine has mold growth. (Correct by washing, rinsing and sanitizing.) *Unapproved bug spray used by owner in between visits from pest control operator. Noncritical violations found: 9.

Jimmy John’s Gourmet Sandwiches, 322 E. Walnut; Critical violations found: 3. *General lack of hand-washing at appropriate times. Employee counted money with bare hands and put on gloves without washing hands, employee swept floor with bare hands and put on gloves without washing hands, employee talked on phone and wrote down orders with bare hands and then put on gloves without washing hands. *Employee made up quat sink in usual manner but concentration did not measure on test strips – had to add five times more to get up to 200 ppm. *Quat concentrate in unlabeled container, no cap. Noncritical violations found: 2.

Kickapoo High School, 3710 S. Jefferson; Discussed importance of date marking consistently. Also informed custodial management to keep dumpster lids closed at all times.Critical violations found: 0. Noncritical violations found: 1.

Los Portales Authentic Mexican Restaurant, 1036 W. Sunshine; Reinspection. Corrections: Manager showed copy of bill from pest-control company. They were out the day after the inspection. Many spring traps observed and glue boards, several mice have been caught in back room area. Raw meat now stored below ready-to-eat foods in walk-in, white cheese sauce is kept in walk-in until ready to use. Critical violations found: 1. *Potentially hazardous foods are not cooled according to times and temperatures in food code when 6” deep pans of beans cooked yesterday are still 55-60 F, 2-4” deep pans were still above 50 F. (All discarded. Using ice bath according to manager but discussed using 2” pans and leaving beans uncovered up on top shelf of walk-in. Left temperature/time log sheet and will return on 10/18/06 to check.) Noncritical violations found: 0.

Los Portales Authentic Mexican Restaurant, 1036 W. Sunshine; Reinspection. Corrections: Manager kept cooling log when beans were cooled yesterday, beans in walk-in 44 F. Took six hours to cool them down. Encouraged to keep log periodically to make sure food cools in the proper time frame. Nothing thawing in the sink at this time. Critical violations found: 0. Noncritical violations found: 0.

McDonald’s, 3250 E. Chestnut; Reinspection. Critical violation corrected, fly problem has been eliminated. Critical violations found: 0. Noncritical violations found: 0.

Mexican Villa South, 2755 S. Campbell; Critical violations found: 2. *Chili meat on steam table 115-117 F. Water on steam table 128 F. Unclear if this was properly reheated to 165 F. Allowed to reheat due to short time frame. Restaurant is currently without a steamer and having to reheat with microwave. *Cutting gloves found above sink unclean. Noncritical violations found: 2.

Ott’s Pasta Carry Out, 1437 E. Cherry; Critical violations found: 0. Noncritical violations found: 3.

Popeyes Chicken & Biscuits, 1725 W. Sunshine; Could not locate permit to take, must be posted on site. Permit suspended. Critical violations found: 2. *Inside of ice machine moldy (Empty ice, remove shield, clean and sanitize). *No hot water available on site. (Hot water heater must meet peak demand.) Replaced with minimal water heater for temperature fix without discussion with health department. Noncritical violations found: 1.

Popeyes Chicken & Biscuits, 1725 W. Sunshine; Reinspection. Critical violations corrected; permit reissued. Critical violations found: 0. Noncritical violations found: 0.

Ryans, 1950 E. Kearney; Critical violations found: 3. *Chicken legs on buffet were 103 F at 2:40 PM. Discarded promptly. (Must be held at 140 F or above, or use time as a control and document.) *Sour cream was 47 F in cold hold (Switched light bulb over it to a flourescent from a heat lamp) and was at 42 F at 3:45 PM. (Must be held at 41 F or below, or use time as a control and document.) *The insides of ice bins were not clean. (Must be cleaned and sanitized more frequently.) Noncritical violations found: 2.

Ryans, 1950 E. Kearney; Reinspection. Ice bins were clean at this time. Restaurant is now using time as a control and documenting with a grease pencil on sneeze guard for some items. Sour cream was at acceptable temperature. Critical violations found: 0. Noncritical violations found: 0.

Scramblers, 3653 E. Sunshine; Critical violations found: 2. *Two cooks refrigerators not cold holding adequately – 58 F. (Must cold hold at 41 F or below.) *Food not cold holding in coolers adequately (Food discarded – $100 value.) Noncritical violations found: 7.

St. George’s Donuts, 3628 E. Sunshine; Critical violations found: 0. Noncritical violations found: 5.

Steak-N-Shake, 1550 S. Glenstone; Critical violations found: 3. *Ice machine ice chute not clean, specks in drink ice. (Clean and sanitize.) *Two ice bins not clean. (Clean and sanitize.) *Dishwasher sanitizer is weak. (Must be 50-100 ppm chlorine solution.) Noncritical violations found: 4.

Stillwell Satellite Center, 525 S. Campbell; Food catered in by South Side Senior Center. Critical violations found: 0. Noncritical violations found: 0.

Taco Bell, 601 S. National; Critical violations found: 0. Noncritical violations found: 0.

Hultgren Grocery, 7078 W. Hwy. EE, Springfield; Critical violations found: 0. Noncritical violations found: 2.

Price Cutter Express, 5505 S. Hwy FF, Battlefield; Critical violation was corrected at the time of the inspection. Critical violations found: 1. *There is mildew growth inside the soda nozzles. (Nozzles were removed and cleaned.) Noncritical violations found: 2.

Sonic Drive In, 411 Miller, Willard; Reinspection. Critical violations noted on the previous inspection have been corrected. Critical violations found: 0. Noncritical violations found: 0.
