CITY RESTAURANT HEALTH INSPECTIONS

Listed here are the restaurant health inspections conducted by the Springfield-Greene County Health Department. Restaurants are inspected for critical and noncritical violations of state standards. According to the Springfield-Greene County inspection code, critical violations are “more likely than other violations to contribute to food contamination, illness and/or injury.” And noncritical

violations, “although important to the overall food hygiene of the establishment, do not

necessarily present an immediate threat to food contamination, illness and/or injury.” The

number of both critical and noncritical violations are given and specific critical violations are listed. *Denotes a critical violation.

Casey’s General Store, 4124 W. Chestnut; Critical violations found: 2. *Inside of ice machine chute and ceiling area moldy (Empty ice machine out and clean and sanitize). *Flies in kitchen (Use pest control measures to rid premises of pests). Noncritical violations found: 1.

Churchill Library Café, 4653 S. Campbell; Critical violations found: 1. *Flies are present throughout the dish room and the prep areas (pest control measures must be in place to effectively rid premises of flies). Noncritical violations found: 5.

Domino’s, 901 S. National; No critical violations. Critical violations found: 0. Noncritical violations found: 0.

Domino’s Pizza, 430 W. Kearney; Critical violations found: 1. *Bottle of cleaner is stored over open top bottled sauces (toxic materials must be stored in such a way as to prevent possible contamination of food. Corrected immediately). Noncritical violations found: 1.

Downtown Market Deli, 707 S. Campbell; Critical violations found: 0. Noncritical violations found: 0.

Expresslane, 3905 W. Chestnut; Critical violations found: 1. *Corn dogs at 133 F in hot hold, cheeseburgers at 126 F in hot hold (Potentially hazardous foods must be held hot at 140 F or above). Decided to use time as a control – will check temps at beginning when first cooked serve only from 10 a.m.-2 p.m. Everything thrown away at the end of that time. Noncritical violations found: 4.

Expresslane, 3905 W. Chestnut; Reinspection: Using time as a control – witnessed documentation. No violations.

Frisco Crossing, 3253 E. Chestnut; No violations.

Frontera Mex-Mex Grill, 3524 S. National; Critical violations found: 6. *Employees not washing hands. Requires hand washing after eating, after handling raw meats or other contaminates and before using gloves. *Employee eating meal in dishwashing area. Employees cannot eat in areas where food, utensils or equipment are stored, prepared or cleaned. *Employee has open drink cup in kitchen. Requires sealed drinking containers only. *Employee preparing food at grill was not wearing gloves properly. Must use gloves or utensils on ready-to-eat foods, not bare hands, and must change gloves and wash hands after handling raw meats or other contaminants. *Several ready-to-eat foods in refrigerator were not marked with a “use-by” date. Requires ready-to-eat foods held for more than 24 hours to have a “use-by” date marked on item. *Utensils on back preparation table and magnetic strip were stored unclean. Must wash, rinse and sanitize utensils before storing. Noncritical violations found: 4.

Frontera Mex-Mex Grill, 3524 S. National; Reinspection: Critical violations were corrected. Date marking was discussed, owner stated they make most items fresh daily, however they have begun date marking prepared items held over 24 hours. Employees were given education regarding proper use of gloves and were using them appropriately. Utensils were stored clean. Employees were given education as to the inappropriateness of eating and drinking in the kitchen. Employees were given education regarding proper hand washing. All noncritical violations were corrected. No violations.

Golden House, 2744 W. Chestnut; Permitted. No violations.

Hardee’s, 3465 W. Chestnut; Discussed proper hand washing and glove use when working with raw and ready-to-eat products, grill area sanitizer must be changed more often. Critical violations found: 4. *Employee used gloved hand to prep raw sausage and then used same gloves to dispense cooked egg into pan (Glove must be changed between tasks to prevent cross contamination. (Eggs discarded immediately). *Egg mixture at 55 F on counter, sausage at 114 F off of the heat source (Potentially hazardous food must be held cold at 45 F or below or held hot at 140 F or above. Sausage reheated to 165 F, eggs used immediately only stay out on counter for one hour). *Dirty knives put into clean rack (Utensils must be washed, rinsed and sanitized before and after use or at least every four hours when in contact with potentially hazardous foods. Knives washed, rinsed and sanitized immediately) *Sanitizer bottles set on shelf in front of open single service items and on rack full of baking pans (Chemicals must be stored to separate them from food and equipment. Bottles moved immediately). Noncritical violations found: 4.

Hardee’s of Springfield, 1444 E. Battlefield; Reinspection: Inside of ice machine was clean. Dipper well was operating properly at this time. Critical violations found: 0. Noncritical violations found: 3.

Hot Shots Espresso, 1510 N. National; No violations.

Jump Stop Grill, 2540 N. Glenstone; Critical violations found: 3. *Two dead flies on counter used for food preparation. (Must clean and sanitize food-contact surfaces as needed.) Cleaned and sanitized prior to exit. *Numerous flies in food preparation area. (Must take actions to minimize flies.). *Spray bottles of cleaners and Easy-Off were being stored on or above food preparation areas. Promptly relocated. (Must store cleaners in a manner to prevent possible food or food-contact surfaces from contamination.) Noncritical violations found: 6.

Kum & Go No. 473, 620 N. National; Reinspection: Critical violations were corrected. Soda nozzles were still moldy, cleaned and sanitized immediately, will clean daily. Breakfast sandwiches at 158 F. No violations.

Kum & Go No. 463, 425 N. West Bypass; Critical violations found: 2. *Tongs not being washed, rinsed, and sanitized in correct place or in correct time frame (Utensils in contact with potentially hazardous foods must be washed, rinsed, and sanitized at least every four hours and must be done in ware washing sink. Metal holder would also have to be washed, rinsed, and sanitized every four hours and in ware washing sink). *No hot water at 9:30 a.m. (Must have hot water enough for peak demand)(Hot water heater set on wrong setting adjusted. Do not prepare any more food until hot water is back to correct temp and approved to reopen). Noncritical violations found: 5.

Kum & Go No. 463, 425 N. West Bypass; Reinspection: Hot water operational three-vat sink and tongs washed, rinsed, and sanitized Purchase enough tongs and metal cups to be changed out every four hours. Critical violations found: 0. Noncritical violations found: 2.

Kum & Go No. 463, 425 N. West Bypass; Reinspection: Stoppers in place. Three new tongs purchased and a clean Tupperware container in place to ensure protection from contamination. Critical violations found: 0. Noncritical violations found: 0.

Lighthouse Lanes, 350 S. Glenstone; Reinspection: Drink dispenser heads were clean at this time. No mouse poison was found in facility. No violations.

Longhorn Grill, 1645 W. Republic; Reinspection: New cutting boards in place. No violations.

Max Orient, 2825 S. Glenstone; Critical violations found: 3. *Two tubs of cooked chicken were being stored in walk-in refrigerator below boxes of raw chicken. Promptly relocated (Must store raw and cooked meat in a manner to prevent possible cross contamination.). *Fried rice and meat dishes were 120 - 130 F in display case at 10:20 a.m. Promptly reheated (Must be held at 140 F or above.) Need to start steam table sooner to keep food hot. *Cooked rice and garlic in oil were 56 and 60 F respectively on counter top at 10:40 a.m. Promptly refrigerated - out less than one hour (Must hold potentially hazardous foods at 45 F of below.). Noncritical violations found: 5.

Max Orient, 2825 S. Glenstone; Reinspection: Hot hold temperatures were at acceptable levels. Critical violations found: 2. *One tub of cooked rice was sitting below a box of raw chicken in walk-in refrigerator. Immediately relocated. (Must ensure raw meat is being stored to prevent possible cross contamination.). *Bowl of garlic in oil was 60 F (it had been on ice, which had melted and was not replaced). Promptly corrected. Noncritical violations found: 1.

Chestnut; Critical violations found: 2. *Inside of ice machine and soda nozzles in Playplace moldy (Empty out ice and clean and sanitize, wash, rinse and sanitize soda nozzles). *Flies in establishment (Use pest control measures to rid premises of pests). McDonald’s No. 15874, 2115 E. Independence; Critical violations found: 2. *Dishwasher not sanitizing properly (corrected immediately to 50-100 ppm). *Evidence of flies (pest control continues). Noncritical violations found: 2.

Mudhouse, 323 South; Critical violations found: 1. *Employee did not wash hands prior to putting on gloves (employees must wash hands when changing tasks and prior to putting on gloves – policy reviewed). Noncritical violations found: 6.

Roly Poly Sandwiches, 400 South; Critical violations found: 1. *Refrigerators holding potentially hazardous foods were unable to keep food at 41 degrees or below. Lunch meats, cheeses and other items were at 54 to 62 F. Food was discarded. Noncritical violations found: 6.
