The following petitions were filed and relief was granted in the United States Bankruptcy Court in Kansas City for the Western District of Missouri, as a matter of public record. The bankruptcies were received at the federal clerk’s office in Springfield. The bankruptcies appear in order of case, chapter, petitioner and address. City Restaurant Health Inspections

Listed here are the restaurant health inspections conducted by the Springfield-Greene County Health Department. Restaurants are inspected for critical and noncritical violations of state standards. According to the Springfield-Greene County inspection code, critical violations are “more likely than other violations to contribute to food contamination, illness and/or injury.” And noncritical violations, “although important to the overall food hygiene of the establishment, do not necessarily present an immediate threat to food contamination, illness and/or injury.” The number of both critical and noncritical violations are given and specific critical violations are listed. *Denotes a critical violation.

Arby’s, 735 S. Glenstone; Critical violations found: 1. *Sanitizing solution used on counters and tables is too strong (400 ppm). Corrected to 200 ppm quaternary ammonia. Noncritical violations found: 2.

Auntie Anne’s East, 2825 S. Glenstone; No violations found.

Blimpie’s Subs & Salads, 1653 E. Sunshine; Critical violations found: 2. *Two chunks of pastrami have Nov. 17 throw-away date. Today is Nov. 22. Meat is discarded. Once potentially hazardous foods reach the end of their shelf life, they must be discarded. *No sanitizer in two wipe cloth buckets. Buckets dumped and refilled with 50-100 ppm chlorine. Noncritical violations found: 2.

Casa Rico, 1700 S. Campbell Ave.; Critical violations found: 2. *Cooked ground beef is 48 F in three-door refrigerator. Air temperature in refrigerator is 40 F. Beef was cooked yesterday. Either the meat was not properly cooled or spent too much time out when prepping for lunch. Potentially hazardous foods must maintain 41 F or less during cold storage. *No dates on cooked ground beef or cooked beans. Potentially hazardous ready-to-eat foods must be dated. Can keep up to seven days if 41 F or less. Noncritical violations found: 2.

Casa Rico, 1700 S. Campbell Ave.; Reinspection. Corrections made: owner showed dating on prepared foods and cooling measures being used. No violations found.

Chopsticks Restaurant, 1128 S. Glenstone; Critical violations found: 3. *Hands not washed before glove use (educated staff). *Smoking inside back door of kitchen (educated staff where they could smoke). *Inside of ice machine deflector not clean (clean and sanitize). Noncritical violations found: 5.

Cody’s No. 5, 3015 W. Republic Road; No violations found.

Cox North-Emerald Gardens Cafe, 1423 N. Jefferson; Critical violations found: 3. *Chili placed in hot holding unit prior to reheating to 165 F. Corrected. *Gravy and eggs on steam table below 140 F or above. If time is used as control, keep time and temperature log. Corrected. *Moldy cream cheese in front-line refrigerator belongs to personnel. Keep personal items labeled and separate from items to be served. Corrected. Noncritical violations found: 1.

Evangel University Cafeteria – Crusader Hall, 1111 N. Glenstone; Critical violations found: 1. *Middle compartment of three-vat sink has a leak. (Must repair drain.) Noncritical violations found: 2.

Evangel University Student Union – The Joust, 1111 N. Glenstone; No violations found.

Food 4 Less, 313 E. Battlefield; Reinspection; Corrections made: repackaged sliced meat given seven-day shelf life. Received information from pumpkin pie manufacturer concerning shelf life, and pies dated accordingly. Employees wearing ball caps. Ingredient labels added to fruit pies. No violations found.

Garbo’s Pizzeria Midtown, 307 S. National; Advised manager to put on sweep for back door. Critical violations found: 2. *Leak under two-hand sink (maintain in proper repair). *Evidence of gnats (continue pest control). Noncritical violations found: 1.

Great American Cookie Co. kiosk, 2825 S. Glenstone; Reinspection; Corrections made: opened carton of ice cream mix dated. Critical violations found: 1. *Employees unaware of how many days to give opened ice cream mix. Employee unable to describe the three steps used in cleaning and sanitizing blenders. Noncritical violations found: 1.

Harter House Bakery, 1500 E. Republic; No violations found.

Harter House Deli, 1500 E. Republic; Critical violations found: 1. *Employee arranging shrimp platter with bare hands. Requires no bare-hand contact with ready-to-eat foods. Corrected by using gloves. Noncritical violations found: 1.

La Quinta Inn, 1610 E. Evergreen; Critical violations found: 1. *Juice dispenser heads not clean. Noncritical violations found: 6.

Maranatha Community Center, 304 W. Bethany; Critical violations found: 1. *Hot sanitizing dishwasher not reaching 160 F on equipment. Noncritical violations found: 1.

Mr. Yen’s, 4117 S. National; Reinspection; Critical violation was corrected. Dipper wells holding scoops were repaired, and in-use scoops stored under running water. No violations found.

Springfield Golf & Country Club Snack Bar, 3106 E. Valley Water Mill; Reinspection; Ice scoop has been replaced. No violations found.

Study, Harry P., School, 2343 W. Olive; Dishwasher sanitizing properly and refrigeration is working good. No violations found.

Sub Shop, 1721 E. Seminole; No violations found.

Subway Sandwiches, 930 N. Glenstone; Critical violations found: 2. *Food employee was eating in food preparation area. (Must ensure employees are not eating in food preparation areas.) *A few containers of cleaners and medications were being stored above food or food preparation areas. Promptly relocated. (Must store cleaners and medications in a manner to prevent possible food contamination.) Noncritical violations found: 0.

The Repair Shop, 2251 E. Kearney; Critical violations found: 4. *Some ground beef was not reheated to 165 F or above before being placed on steam table. (Must be reheated to 165 F.) *Ten baked potatoes were 89 F at 3 p.m. Immediately discarded. (Must ensure baked potatoes are kept at 140 F or above or cooled to 41 F.). *Cooked pinto beans and chili in refrigerator were not labeled with “use-by” dates. (Must label refrigerated, ready-to-eat, potentially hazardous foods with “use-by” dates.) *Budweiser taps and one soft drink dispenser gun were not clean. (Must clean and sanitize more frequently.) Noncritical violations found: 5.

The Tamale King, 1342 W. Sunshine; Discussed reheating food in microwave and went over cooling, hot-holding temperatures. Will bring handouts. Gave “wash hands” sticker for restroom. Critical violations found: 1. *Meat cooked for tamales not cooled properly. Potentially hazardous foods must be cooled to 41 F within six hours. Discussed ways to “quick chill” meat. Will revisit to check new procedures. Noncritical violations found: 7.
