Chestnut Expwy.; 82; *cooked mushrooms were 120 degrees F at 10:15 a.m. (immediately reheated to 165 F), ice scoops lying on top of drink dispensers, doors on refrigerators not working properly, the inside of microwaves and drink dispenser heads were not clean, the following non-food contact surfaces not clean: counter tops under equipment, shelving in walk-in refrigerator, inside of one refrigerator, sides of grill, inside of revolving oven and underside of drink dispensers and some cabinets, cup dispensers overfilled exposing mouth contact surfaces, a few bags of foam and plastic food containers being stored on the floor, employee hand sink is badly cracked, area around employee and women's hand sinks were not clean, outside dumpster had both lids open and was half full of trash, the area around dumpster was not clean, the floors in food prep and storage areas including walk-ins were not clean, some ceiling tiles were missing or damaged in food prep and storage areas, the walk-in freezer and refrigerator walls are in poor repair, four lights in food prep areas are missing or have damaged shields.
