Independence; 83; *potentially hazardous food in hot box was 121 degrees F, 123 degrees F and 125 degrees F, *north hot well had chicken mix 124 F, shredded beef 120 F, ground beef 120 degrees F, meat sauce 132 degrees F (all reheated to 165 degrees F), food not covered in hot well, sneeze guards for taco bar broken, inside of ice machine not clean, tortilla ball tray holder not clean, bar machines dusty, freezer floor covered with ice, beer walk-in moldy, *hot water at front wait station 68 degrees F, both hot and cold water just trickle out, no towels at bar hand wash sink.

West Bypass; 89; a number of food containers on steam table were not covered or protected by sneeze guard in a high traffic area, counter by front drink dispenser has water damage, *drain on prep sink is leaking onto kitchen floor, *access to employee hand sink was blocked (corrected promptly).

The Inferno; 220 S. Campbell; 96; no drain boards on three compartment sink back room downstairs, floor in back room has missing tiles, walls and ceilings in back room must be smooth and easily cleanable.

Airport Commerce Ave., Springfield; *lack of food safety knowledge didn't know reheating procedure, dating procedure or required hot holding temperatures; *hand sink full of dirty dishes, had to wash hands in three-vat sink (cleaned and corrected); *shrimp in plastic containers left over from yesterday being placed in hot box without proper reheating (thrown away); *some items dated with make date, some items dated with use by date confusing (should have use by date); *several items not dated sliced ham, leftover hamburgers and opened cottage cheese; not enough area to air dry dishes due to location of hot plate (moved to a different location); pop lines run along floor making it impossible to clean.
