Ebenezer Group Home; 7331 N. FR 137, Springfield; no chlorine test strips, no 0-220 degrees F food thermometer, no thermometer in refrigerator, employee restroom not usable, no handle on stool, handicapped restroom water turned off at one faucet due to leak, Formica damaged around the kitchen sink.
