Indonesia's National Transportation Safety Committee released a final accident report on the Lion Air flight that crashed and killed 189 people a year ago.
The report found design flaws with the Boeing 737 Max jet, as well as inadequate training and maintenance problems.
The aircraft had problems on its last four flights, including one the day before the fatal crash, according to the report.
