Diabetes CARE will be part of the CoxHealth Diabetes Center, which has about 3,581 Medicaid and 708 self-pay patients who have diabetes and are at risk for complications. The project is expected to reach about 50 percent of them, according to a CoxHealth news release.
Diabetes experts will train patients in self-management, as well as help high-risk individuals develop a program to prevent diabetes-related problems. Those high-risk patients will have an initial assessment with a nurse, licensed professional counselor and registered dietitian; they then will have ongoing meetings with the specialists to help with behavior changes, setting goals and finding medical resources.
The grant is from the Missouri Foundation for Health and was obtained through the CoxHealth Foundation.
